Heart of Belonging Nomination

Every community has individuals whose quiet acts of kindness, service, and bridge-building make it stronger. They may not seek recognition, but their impact is lasting. We invite you to nominate someone whose life reflects the Heart of Belonging. Selected nominees will be honored by having their initials incorporated into a collaborative artwork created by hundreds of Minnesotans and unveiled on September 6, 2026, during Many Rhythms. One Celebration at the Minnesota State Fair. Nominations close August 3, 2026.

Sustained Commitment (100-150 words) – How has the nominee demonstrated a long-term commitment (generally five years or more) to strengthening their community?

Bridge Builder (100–150 words) How has the nominee brought people together across neighborhoods, cultures, generations, or backgrounds? Please share specific examples.

Outside the Spotlight (100–150 words) How does the nominee serve others without seeking recognition? What makes this person one of the quiet builders of community?

Please provide one or more links that help verify the nominee’s community work. This could include: A social media post about their service A community organization or project webpage A news article, newsletter, or event announcement A public recognition, testimonial, or profile Paste the link(s) below and briefly explain what each link shows. We recognize that many community builders work outside the spotlight. Limited public documentation will not automatically disqualify a nominee; however, the selection committee may contact the nominator or community references to verify the nominee’s contributions.

Nominations will be evaluated by a Community Advisory Board based on four primary criteria:

(1) Sustained Commitment – the nominee has demonstrated a consistent commitment to strengthening their community over several years;

(2) Bridge Builder – the nominee brings people together across cultures, neighborhoods, generations, or backgrounds, fostering understanding and belonging;

(3) Outside the Spotlight – the nominee serves with humility, making meaningful contributions without seeking public recognition; and

(4) Evidence of Contributions – the nomination includes examples or public links that help verify the nominee’s community impact.

The selection process seeks to honor individuals whose quiet, sustained efforts have helped create stronger, more connected communities.
